Child Growth and Development: Milestones & When Parents Should Worry

You may have noticed something, or someone may have commented on your child’s height, weight, speech, or behaviour. Or you may simply feel unsure — and that’s okay.

As a paediatrician, this is one of the most common concerns I hear from parents.

Important to remember:
Growth and development go hand in hand, but they do not always progress at the same pace.

Milestones Are Guides, Not Deadlines

Children reach developmental milestones at different times — and this variation is completely normal.

Some children may talk early while their motor skills develop later. Others may walk or run early but take more time with speech.

What we look for as paediatricians is progress, not perfection. A child who is steadily learning new skills at their own pace is often doing just fine.

Behaviour, Attention & Emotional Development

Today, many parents worry about behaviour, attention span, and emotional development — especially after reading about conditions like ADHD or autism online.

It’s important to remember:

Not every delay or behavioural difference indicates a diagnosis
Many children show normal variations in attention, activity, and emotions
Sleep, nutrition, routine, and environment play a major role

It’s worth discussing concerns with a paediatrician if challenges:

Persist over time
Affect daily functioning or learning
Cause distress to the child or family

Early conversations help us understand whether a child needs reassurance, monitoring, or support — without rushing into labels.

How Often Should My Child Be Weighed and Measured?

Regular growth monitoring is a key part of paediatric healthcare.

Your child should be measured during:

Routine check-ups and immunisation visits
Visits when your child is unwell

Typical schedules include:

Within 1–2 weeks of birth
At 2, 4, 6, 9, 12, 18, and 24 months
Once yearly after 2 years of age and during adolescence

How Is a Child’s Growth Tracked?

Growth is tracked through serial measurements of:

Weight
Height or length
Head circumference (in younger children)

These measurements are plotted on age- and sex-specific growth charts recommended by the WHO and the Indian Academy of Pediatrics (IAP).

Doctors assess trends over time — not single values — while also considering genetics, nutrition, sleep patterns, physical activity, and screen exposure.

Nutrition & Sleep: More Than Just “Eating Enough”

Nutrition plays a vital role in healthy growth and brain development — but it’s not only about quantity.

Protein for muscle and tissue growth
Iron for brain development, energy, and attention
Calcium & Vitamin D for bone strength
A balanced mix of carbohydrates, fats, fruits, and vegetables

Children who are picky eaters or rely heavily on packaged foods may appear full but may still miss essential nutrients.

Healthy growth is best supported by nutritious meals, adequate sleep, regular physical activity, limited screen time, and occasional treats in moderation.

When Should Parents Seek Medical Advice?

Most differences in growth and development are normal. However, seek guidance if you notice:

Growth slowing or stopping over time
Loss of previously achieved skills
Delays in speech, movement, or learning
Persistent feeding or sleep difficulties
Behaviour affecting school or daily life
Ongoing anxiety, withdrawal, or emotional distress

Early evaluation doesn’t mean something is seriously wrong. Often, it brings reassurance — or simple steps that help your child thrive.

Every child grows at their own pace. Comparisons — especially online — can make normal differences feel like problems.

If something continues to worry you, it’s okay to ask. Growth is not about keeping up with others — it’s about helping your child grow steadily, confidently, and well.
